Get Started with Aged Care

Support at Home is the Australian Government's aged care program, designed to help older people remain independent in their own homes. Following an assessment through My Aged Care, eligible individuals may receive funding for services such as personal care, domestic assistance, nursing, allied health and care management. If you're not sure where to start, that's okay. We'll explain your options in plain language, help you understand the process, and support you every step of the way.

Our domestic assistance services help clients with everyday household tasks, including general cleaning, laundry, ironing, changing bed linen, dishwashing, and other light household duties. Transport and shopping

Maintaining access to the community is important for health and wellbeing. We provide reliable transport assistance for medical appointments, social activities, community participation, and personal errands. Our team can also assist with grocery shopping, pharmacy visits, and other essential errands to help clients remain independent and connected.

Social support and companionship

Social connection plays an important role in healthy ageing. Our companionship services provide friendly support through meaningful conversations, shared activities, community outings, hobbies, and social engagement. We help reduce social isolation and support clients to maintain relationships and continue enjoying the activities that matter most to them.

Our qualified nursing team provides professional clinical care tailored to each client's health needs. Services may include medication management, wound care, chronic disease support, health assessments, continence management, and clinical monitoring. We work closely with clients, families, and healthcare professionals to promote health, safety, and quality of life.

Care coordination

Navigating aged care services can be complex. Our care coordination team works closely with clients and families to develop personalised care plans, organise services, monitor outcomes, and ensure support is delivered effectively. We help coordinate care providers, allied health services, and community supports to create a seamless care experience.

Help navigating My Aged Care

Understanding the aged care system can be challenging. We provide guidance and support to help clients and families navigate My Aged Care, understand available funding options, access assessments, and make informed decisions about their care. Our team is here to simplify the process and support you every step of the way.

Assistive Technology and Home Modifications

We help clients access equipment and home modifications that support safety, independence, and daily living. This may include mobility aids, bathroom safety equipment, communication devices, and home modifications designed to improve accessibility and reduce the risk of falls. We work with clients and relevant professionals to identify solutions that best meet individual needs.

Restorative Care and End of Life Pathways
